Position Summary:

The Social Worker II responsible for coordinating the facility placement, community agency referral and social coping aspects for 100% of the assigned patient populations. Serve as a direct provider, referral source and resource to other staff in an effort to promote the delivery of high quality, fiscally responsible continuing care services. Provides professional social work services to patients, families, and /or caregiver(s), which reflect the biopsychosocial needs of the patient and enable him/her to achieve an optimal level of health accordance with social work practice and hospital policy and procedures. Performs other related duties required or directed.

Job Requirements:

- Bachelor's degree in Social Work or related field from an accredited institution. Master's degree is preferred.

- Must have LSW or MSW from the State of Ohio.

- Three to five years' experience in a comprehensive inpatient Psychiatric Unit (or comparable work experience) preferred.

- Certified in Nonviolent Crisis Intervention preferred.

- Strong interpersonal skills to work independently.

- Knowledge of community resources and/or federal regulations.
