The goal of the program is to provide support to individuals who are in isolation due to being at high-risk for contracting Covid-19. This program will provide enhanced case management and service delivery to these individuals in a 24-7 facility that will provide a safe, therapeutic environment for this population. This position will provide direct supportive services and to individuals who are in isolation as part of a rotating schedule, and will work to aid staff in constructively engaging persons served.

CHS uses the Housing First and Harm Reduction service delivery models, assisting clients with housing as the immediate need without prerequisites to abstain from any harmful behavior prior to being housed or receiving assistance. We help clients identify their own goals and support them to discover safer alternatives.

Service Coordination-Participant Involvement

  • Provide individual assessment of participant needs. Assist the individual in meeting their goals. This includes a heavy emphasis on goal planning, encouraging safe behavior for high risk populations, and other supportive services.
  • Coordinate with the treatment team to provide referral services to appropriate referral base, such as drug and alcohol, medical, mental health, housing, vocational, and education.
  • Establish a supportive relationship with persons served to assist them in improving their understanding of themselves, including basic issues like respect for differences, individual uniqueness, creating and maintaining relationships, and values.

    Duties and Responsibilities:

  • Provide overview of room and operation as participants move in.
  • Deliver boxed meals to each room during scheduled meal times.
  • Adheres to isolation and sanitation protocols.
  • Complete daily wellness calls.
  • Develop schedules for staggering people to be able to leave the room and be in open spaces.
  • Develop activities that can reduce the stressors of isolation.
  • Ensure screenings occur and protocol is followed for any participant who may disclose Covid related symptoms.
  • Maintenance of progress notes in the CHS database and creation/maintenance of paper records.
  • Generates statistical and written reports as requested by her/his supervisor.
  • Communicate with the CHS staff, other service providers and landlord/building management as needed and appropriate regarding participant issues.
  • Assist other homeless assistance staff, both management and program, as needed.

Service Coordination-Programmatic Involvement

  • Participate in scheduled CHS staff meetings and other related scheduled meetings.
  • Participate in designated interagency meetings and case conferences.
  • Maintain regular and timely communication with different UPMC Health Plan staff regarding mutual clients and service coordination efforts.
  • Communicate with the CHS staff, other service providers and landlord/building management as needed and appropriate regarding participant issues.
  • Maintain open communication and work cooperatively with other CHS staff to establish and fulfill overall agency and program goals and objectives.
  • Assist other homeless assistance staff, both management and program, as needed.
  • Consult with the director on strategies to improve the effectiveness of the programs, given agency and program mission and working practices.
  • Network and cooperate with external agencies to access appropriate services for all participants of the program.

Minimum Qualifications:

  • Masters of Social Work degree and 2 years of experience in the social service field will be considered. Licensed Social Work certification strongly preferred.
  • Computer literacy with a working knowledge of Microsoft Word, Outlook and Excel.
  • Clinical experience with mental health/Dual Diagnosed/behavioral health/Drug & Alcohol strongly preferred.
  • Experience with helping the homeless preferred.
  • Exceptional organizational, communication and case management skills.
  • Sound interpersonal and problem solving skills.
  • Must have reliable transportation.
