This position is the primary point of contact for patients and referral agencies at UofL Health Peace Hospital. This role entails collaborating and consultation with the interdisciplinary treatment team to advocate for the discharge needs of the patient and link patients to appropriate community resources as available. The Case Manager may assistance unit clinicians and managers in facilitating, organizing, and coordinating clinical functions for inpatient programs. Supports the therapeutic milieu, behavioral health, nursing, and mental health technician staff with group services. Involved in the direct provision of clinical/counseling behavioral health and/or substance use services to patients served by the program. Services include: group therapy, psychoeducation, discharge planning, and coordinating with community, government and/or other behavioral health agencies for aftercare services.

Aids social workers in collection of and review of information from patients, their families, outside sources such as school, EAP & outpatient therapist to assess assigned patient’s needs for discharge resources. Records and communicates information to social worker and/or interdisciplinary treatment team

In collaboration with the social worker and inter-disciplinary treatment team, assists to prioritize assigned patient’s needs for discharge planning resources and documents individualized interventions on assigned patient’s Treatment Plans.

Interacts directly with patient and guardians to participate in the discharge planning as per the Treatment Plan goals/interventions. Coordinates referrals and discharge appointments and collaborates with other service providers and community resources as appropriate.

Maintains accurate documents of discharge plans and collaboration with community resources, patients and others involved in patient’s care.

Maintains an up to date resource center of community based resources for clinical team to utilize for patients/family’s needs

May assist in therapeutic milieu to provide structured interventions to provide psycho-educational groups to patients.

Acts as a patient advocate by ensuring each individual’s rights are maintained at all times.

Stays informed regarding changing status of all patients on the unit. In collaboration with the nursing leadership, assists in providing leadership to maintain a safe and therapeutic environment for patients and staff. Utilizes proactive techniques to de-escalate the milieu and/or individuals.

Participates in development, implementation and utilization of therapeutic milieu activities and interventions.

Required Education, Licensure, and Certification:

- Bachelor’s degree in counseling psychology, psychology, or other closely related health degree with a minimum of 1 ½ years clinical experience specific to unit, program, and field required.
- Able to complete Healthcare Basic Life Support (BLS) training and able to perform BLS techniques required.
-Able to complete Safe Crisis Management training and perform safe crisis management techniques

PREFERRED Qualifications Master’s degree in counseling psychology, psychology, or other closely related behavioral health degree preferred.
