The Memory Support Coordinator / Social Worker is responsible for coordinating all memory care services including supervision and direction of memory care neighborhood staff. Tasks include coordination of resident services, participation in Quality Assurance/Quality Improvement, education and orientation of staff, communication with families, visitors, physicians, staff and other departments leading to completion of resident care. This role requires that 50% of total work hours are scheduled to provide care and services as a Social Worker to our residents.
Qualifications:
* CSWE accredited Bachelor’s degree required
* Master’s degree preferred
* One year experience developing and facilitating activity programs for a specialized memory care program preferred
* One year experience in supervisor role within a health care setting preferred
* Current CPR certification required
* High school diploma or equivalent required
* Ability to assess and effectively communicate with residents
* Demonstrated ability to build and cultivate relationships
Responsibilities and Expectations:
* Knowledgeable about the disease process, the changes associated with memory care illness, how changes affect the individual's ability to function and the adaptive strategies that help maintain a resident's abilities.
* Responsible for assuring residents rights are met.
* Meets with admissions to assess and plan to meet memory care needs.
* Provides supportive counseling to assist the resident and family through the admission process.
* Coaches and enables other staff to respond to resident’s memory care related emotional and/or psychological needs.
* Initiates and maintains relationships with resident and family that positively impact their morale.
* Orients residents and families and assists in adjustment through program involvement.
* Conducts periodic memory care support groups for families, staff and outside stakeholders.
* Completes and maintains a memory care resident history and assessment for each resident upon admission and completes updates when necessary.
* May support the resident admission process by working with the Marketing/ Admissions team. Market PSL services to the community at large; networking, providing tours, lecturing and attending related conferences/events.
* Maintains a current community resource file for memory care education and community resources residents, families and interested individuals.
* Designs and conducts formal/informal education and/or counseling sessions for residents, family members and staff; this may include: intervention skills, re-motivation/behavioral programming, orientation skills, coping skills and other appropriate modalities.
* Actively participates and encourages resident/family participation in the Interdisciplinary Care Planning Conferences.
* Timely completion of the resident UDA forms.
* Provides input into the development of resident Interdisciplinary Care Plans.
* Makes daily rounds to assess quality of care performance, teamwork and quality service.
* Provides verbal reports of resident conditions to Interdisciplinary management.
* Conducts monthly neighborhood meetings regarding planning, policy changes, program changes, education and resident care issues.
Social Work Expectations:
* Meets with resident/family upon admission to assess and plan to meet psychosocial needs.
* Coordinates discharge planning process and arranges for resident services after discharge.
* Actively participates in the Interdisciplinary Care Planning Conferences.
* Completes appropriate documentation upon admission, transfer, discharge, and completes ongoing progress notes.
* Coaches and enables other staff to respond to resident’s emotional and/or psychological needs.
